  • Even as the weather turns colder, it’s still a wonderful time to discover all the hidden gems Canada has to offer. For Stephanie, one of our nurses, her gem is her hometown of Limoges, Ontario. Nestled in eastern Ontario, Limoges boasts Canada’s largest waterpark and is famously known as Ontario’s mosquito capital, but for Stephanie, what makes Limoges really special is dog mushing. “Our Forest Larose is where you will find myself and my family during the winter months enjoying the sport of dog mushing in our numerous mushing trails and mountain bike trails.” Have you ever visited Limoges? #CareLivesHere #SEHealth #PeopleEverything #HopeAndHappiness #MoreThanYouThink

🌟 At Last, Today the Announcement Became Reality … A Global Turning Point for Equitable End-of-Life Care: Our New Partnership with Hospice UK We’re thrilled to announce a major milestone for the SaintElizabethFoundation | SE Health: our new partnership with Hospice UK. At the announcement event, the room was absolutely jam-packed, and the energy was electric, a powerful reflection of the excitement surrounding this collaboration and the shared belief in what we can achieve together. For years, we’ve been learning from communities across Canada, especially through JourneyHomeHospice, our specialized program for people experiencing homelessness and other structural vulnerabilities. This work helped shape our Canadian Centre for Equity and Innovation in End-of-Life Care, created to re-imagine hospice through compassion, dignity, and equity. But the challenges we face in Canada are not unique. Around the world, hospice leaders are asking the same urgent questions: – How do we reach people excluded by traditional models? – How do we adapt hospice for complexity, trauma, and inequity? – How do we ensure dignity and care for everyone, everywhere? That’s why this partnership matters. 🌟 Together with Hospice UK, we’re sharing innovations, challenging assumptions, and co-creating models of care that ensure no one is left behind, anywhere. 🌟 This collaboration strengthens an international movement toward more inclusive, community-rooted, and equitable hospice care. 🌟 It reflects our shared commitment to re-imagining what end-of-life care can and should be. We’re honoured to work alongside an organization internationally recognized for its leadership and bold vision. Together, we’re taking a historic step forward in shaping hospice care globally, and we couldn’t be more energized by the journey ahead. #HopeAndHappiness

🤝 Patient engagement is everywhere. Authentic partnership? That's harder to find.    Across Canada, people with lived experience are leading transformative work in integrated care. They're building national networks, shaping research partnerships, and creating the infrastructure needed for genuine co-leadership. Healthcare systems increasingly recognize the importance of this work, and many are doing more patient engagement than ever before.    What's needed now is the shift from involvement to authentic partnership, where power is genuinely shared. This means moving beyond consultation to co-leadership, where people with lived experience don't just inform decisions but shape them. It requires confronting power dynamics, investing time in building trust and relationships, and changing how systems operate, not just how they engage.     Join us for IFIC Canada's upcoming virtual community - "Partnering with People with Lived Experience to Enable Person-Centered Integrated Care" on December 9th from 12:00-1:30 PM ET.    Our speakers will share what authentic partnership looks like in practice, the foundational work it requires, and how to build on the transformative leadership already happening across the country. Come learn and unlearn together with others working toward integrated care that's genuinely shaped by the people it serves.    Featuring Judy Birdsell, Zal Press, Seeta Ramdass Justine Giosa, PhD, Emily Cordeaux, and Isra Amsdr. Moderated by Jodeme Goldhar and Walter Wodchis.   Register now: https://lnkd.in/gpWtyrzG

What an incredible experience to present at Hospice UK Conference 2025! Nancy Lefebre and I were thrilled to share the SaintElizabethFoundation | SE Health Model of Care and the Canadian Centre for Equity and Innovation in End of Life Care, all born from lived experience supporting patients and families at our specialized program, JourneyHomeHospice. We loved exchanging ideas and hearing about the amazing, specialized work happening across the UK. The conversations were energizing, inspiring, and a reminder of how much passion there is for doing things differently. It was standing room only, filled with curiosity and enthusiasm, and we appreciated being part of a community that is learning and leading together. Feeling grateful, excited, and ready for what comes next. #Leadership #Innovation #PalliativeCare #EndOfLifeCare #HealthEquity #SEHealth #JourneyHomeHospice #Collaboration #GlobalLearning
