Partners manages care in Burke, Cabarrus, Catawba, Cleveland, Davidson, Davie, Forsyth, Gaston, Iredell, Lincoln, Rutherford, Surry, Stanly, Union and Yadkin Counties. Diabetes Awareness Month is a prime opportunity to explore new insights and supports for this pervasive condition. Understanding that members with chronic conditions like diabetes are more prone to struggle with loneliness can be a first step in helping them rise above additional health risks. Serious illness, understandably, can be a very isolating experience resulting in sustained loneliness that is associated with many other barriers to care such as: • Poor mental health • Poor health literacy and med adherence • Limited access to resources, like proper nutrition and transportation to appointments These challenges make it harder for chronically ill members, like diabetics, to adhere to treatment plans, reach out for help from their care teams, and even trust their providers. When we’re able to make meaningful connections with these members to alleviate loneliness, we increase our chances of finding and solving these issues before they negatively impact their health.
