  • Another great Day of Service for the CCA team! Whether assembling home cleaning kits for families entering transitional housing, building beds for children who need a warm place to sleep, decorating cards for homebound seniors, or sorting donations for families in need—our team came together last week with pride and purpose to support our Boston-based communities. Thank you to everyone who participated. Your dedication to our mission is inspiring! And thank you to our amazing community partners: Community Servings, A Bed for Every Child, Heading Home, and Cradles to Crayons. #CCACommunity #Volunteering

September is National Recovery Month, a time to honor the strength of individuals on the path to recovery from substance use and mental health conditions. It’s also an opportunity to raise awareness about the effectiveness of treatment and the vital role that strong support systems play in long-term healing. While recovery can be life-changing, access to timely and appropriate care isn't always guaranteed. Gaps in services can lead to setbacks or hospitalizations, putting a person’s health and future at risk. That’s why I’m incredibly proud of the work we do at Marie’s Place, Commonwealth Care Alliance's community-based respite unit. A heartfelt #thankyou to our compassionate and dedicated interdisciplinary team. Your commitment to health, safety, healing, and helping individuals reintegrate into their communities truly changes lives. Because of your tireless efforts, more people have access to the personalized care and support they need to not only recover—but thrive. #recoverymonth #mentalhealthmatters #CommunityCare #CCACommunity

Primary Care Director | Family Nurse Practitioner | Passionate about serving the underserved

ADA Anniversary  July 26 35 years ago today, the Americans with Disabilities Act marked a turning point for disability rights in the U.S. This landmark legislation has expanded access, reduced discrimination, changed public perception, and empowered individuals with disabilities to participate more fully in society. I’m proud to serve as Director of CCA Primary Care, a practice designed to meet the unique needs of individuals with physical and developmental disabilities.  Here’s how we’re improving care for those who need it most: ✔️ Accessible clinics with specialized equipment like wheelchair scales, stretchers, Hoyer lifts, and slide boards ✔️ Flexible care: in-clinic, at home, or virtual, based on patient needs ✔️ Longer visits when needed for complex care ✔️ Interdisciplinary teams: APCs, physicians, nurses, behavioral health, and support staff ✔️ Focus on preventing disability-related complications through early, comprehensive care We believe healthcare should meet people where they are—physically, emotionally, and medically. #ADA35 #DisabilityInclusion #AccessibleHealthcare #PrimaryCare

  • CCA’s ability to support individuals with complex needs is strengthened by dedicated partners like Upham's Community Care, which provides high-quality, integrated healthcare that combines care management with primary, specialty, and urgent care. Their holistic, family-centered approach goes beyond medical treatment to address social needs such as housing, legal support, immigration, child support, insurance, and emergency assistance. Services are culturally and linguistically tailored to honor the values, needs, and preferences of their patients. Upham’s comprehensive family approach supports individuals at every stage of life—from pediatric care and developmental screenings for infants to geriatric care that promotes aging with dignity. They're also dedicated to reducing the impact of substance use, reflected in staff training on key topics—overdose prevention, naloxone administration, and pain management—along with ongoing efforts to expand capacity for compassionate, comprehensive opioid use disorder treatment. “As a pillar of the Dorchester community, Upham’s Community Care has shown unwavering dedication to meeting patients and families where they are,” said Erik Helms, CCA’s SVP of Provider Contracting and Performance. “They’ve been a trusted partner to CCA, helping improve the health and quality of life of our members for over two decades.” #ThankYou, Upham's Community Care, for your passion and commitment to serving our members. #IntegratedCare #CommunityCare #CCACommunity
